'Little Mac' isn't a stage name. His first name is, literally, Little. This is confirmed by his birth certificate.
Little Michael is his name and his mother's accent (she Italian) turned Mike into Mack and it just kind of stuck, so when he needed a stage name he chose what he'd always been: Little Mac.
