This is a straight forward step by step on how to actually shift, this is how I do it.
Grounding - Ground yourself in your CR (current reality), the reality you wish to shift from. Ground the physical body you are currently experiencing (CR body). Do this by appreciating your day so far (look back on it and remember everything you did in your CR today). This is something that Neville Goddard did, he would visualize his day from when he got to bed, to when he woke up this morning. Appreciating your day is simple but very effective. The universe loves when we appreciate what we have, and we align ourselves in a state of receiving and gratitude. I also see it as a thank you to this current reality before you leave.
Now you have grounded your body, and you know how to ground yourself for when you get to your DR (desired reality) body.
SATS - This is an abbreviation for the State Akin To Sleep. This state is when your physical body is asleep, but your mind/awareness remains awake. The way I get into the SATS, is by getting comfy and pretending my body is asleep. I like to do this in the early morning, when I am still a bit tired, that way it's easier for my body to fall back asleep. I listen to a meditation, audiobook, or nothing at all. I focus on my breath, and nurse my body to sleep. Once I feel my body go numb, get heavier and feel myself enter this dream-like state, I know I am in the SATS.
Void - The void is a place where time and space do not exist. When you're there, you are no longer a body, but only awareness in complete nothingness. When I enter this state, it is usually after I've entered the SATS. Once in SATS, I stay there and let my mind wander a bit, then it sort of transitions into the void. I like to think of the void as the in between, the place where the action of shifting happens. In the void, this is when I get shifting symptoms; my heart beats all over my astral body, I feel vibrations go through me, I feel like I'm flying or zooming in outer space... This is the place I am in before my desired reality, think of it as a gas station to your desired location.
Once you are in the void, you can affirm present affirmations that you have shifted and that you are presently in your desired reality. Think of it as if you're already there; feel your surroundings, use your five senses, think thoughts that your desired reality self would think, etc.
Warning: The SATS, void and shifting symptoms can be quite frightening. It took me many tries to get used to them. If you are scared when these things are happening, remember that it is completely normal to be afraid and you can leave whenever you want. You do not need to rush yourself!
Desired Reality - This part can be a little difficult. Sometimes we can start to get excited and then shift back (at least that's what happened to me!). The key is to stay calm and let it happen. Feel yourself as though you are in your desired reality body, feel your heartbeat and breath, touch the bedsheets you lay on, hear your comfort character calling out your name, think of how you're really here and how you've made it, smell it, etc...

Remember that reality shifting (this is what I believe anyway) is transferring your consciousness to a different reality. You are totally capable of doing it!
